Booklet has soiling and staining to the cover and pages. The red cover has an uneven red. The cover is attached to the booklet by the top staple only. No other loose pages to booklet. The first page has been tore out and the second page has a number written on it in pencil. There is another page that has writing in the booklet and there are also large brown stains to that page area. Booklet has out edge staining. Booklet has been lightly folded vertically at one time. There are various creases to the pages.
